New Patient Forms

As a new patient to the practice, it is helpful to fill out a New Patient Health History form and a Notice of Privacy Practices form prior to your appointment. Both forms may be brought to your scheduled appointment.

Also as a new patient, it is important to arrive 15 minutes early to your appointment to assure adequate time for registration. It is also important and to bring your current active insurance card to each appointment.

Allergy Documents

If you are scheduled for allergy testing, please review the packet below for preparation, medications to discontinue prior to testing, and a questionnaire to complete and bring in to the skin testing appointment.

Ultrasound and FNA Documents

If you are scheduled for a thyroid or neck ultrasound or a fine needle aspiration (FNA), the following documents are helpful in preparation for your scheduled appointment or procedure.

Sinus Documents

The Sino-Nasal Outcome Test, or SNOT-20 form, can be beneficial to complete and bring to your initial new patient evaluation if you struggle with allergies or sinus problems.
